24-Hour DSPD Community Placement ORS Referral Form
Parents whose children are placed in 24-hour care with the Division of Services for People with Disabilities (DSPD) are required by law to contribute financial support toward their child's care. When the child is first placed into care, DSPD staff should complete the 24-Hour DSPD Community Placement Referral Form and fax the completed form to 801-536-0378 or mail the form to:

The 24-Hour DSPD Community Placement Referral Form notifies the Office of Recovery Services (ORS) of the child's placement into 24-hour care and provides the necessary information to open a case for the child. Providing this form to ORS when the child is first placed into care enables ORS to establish the support obligation in a timely manner, which helps minimize past-due support.
