How to Apply for Child Support Services
The following forms have been made available for print using Adobe Acrobat Reader. Click here to download the latest version of Adobe Acrobat Reader. Please DO NOT use Acrobat PDF Writer to fill out the forms below. These forms cannot be returned electronically, the Office of Recovery Services is required to receive applications with original signatures.
1. Read the Notice of Services (ANIA form) carefully; ANIA
2.Complete and sign the following forms (you must print the forms and mail them in. They cannot be filled in on-line.)
- Parents' Background information (AIIA form) AIIA
- Statement of Arrearage Claimed (AIIC form) AIIC
- Application for Non-IV-A Services (ANIB form) ANIB
3. Send in
- A copy of your support order(s) and any
modifications - Proof of your monthly income
- A copy of the child's birth certificate if you were not married when the child was born
If you wish to open more than one case, you need to complete a separate set of forms for each non-custodial parent.
